Registration Form

This year teams can bring 5 players. Substitutions will be allowed on the fly during games. This will keep the level of play up (more energy!) and if a player gets injured on a team they’ll still have 4 players.
Each player must be 18 years or older by the date of the tournament to participate
Registration is currently open to returning teams only
Registration opens to everyone on November 10 on a first-come first-served basis. The first 24 teams to register will be guaranteed a spot. Your team is considered registered ONLY after payment in full has been received.
Team fee is $800
Early bird special: Team fee is reduced to $750 if paid by December 12, 2011
Payment must be received by December 12, 2011 for the early bird discount, and all payments must be received by January 5, 2012. Late payments will not be accepted – your spot will be filled if not paid on time, no exceptions.
Four or Five person teams must PAY TOGETHER as a team, the captain is responsible for paying/delivering the team fee to Chad Laszlo, the tournament registrar.
